Form SB-3 (Application for Opening of Account) is the standard composite form for opening SB, RD, TD, MIS, SCSS, and KVP accounts, submitted along with the mandatory Customer Information Form (CIF/KYC) and self-attested Aadhaar and PAN documents.

Form SB-103 is the official Pay-in Slip / Deposit Voucher used across all Head, Sub, and Branch Post Offices to deposit cash or cheques into savings, recurring deposit, monthly income, and public provident fund accounts.

Form CN22 is required for international small packets and letter mail with contents value up to 300 SDR (approx ₹30,000). For articles exceeding 300 SDR or parcels weighing over 2 kg, the detailed Form CN23 (Customs Declaration) and CP71 (Dispatch Note) must be attached.
